Health Insurance
• Employee portion of premium paid via payroll deduction (pre-tax)
• Incentive for PPO utilization

Life Insurance
• 1 X annual salary Life Insurance ($ 50,000 max)
• Includes Accidental Death & Dismemberment (AD&D) Insurance
• 100% of premium paid by company

Long Term Disability Insurance
• 100% Premium paid by company

Flexible Spending Plan
• Pre-tax dependent care reimbursement
• Medical expense account for pre-tax reimbursement

Voluntary Benefits (employee pays premium via payroll deduction)
• Term Life Insurance (can add spouse/dependents)
• Short Term Disability
• Cancer Policy (can add spouse/dependents)
• Accident Insurance (can add spouse/dependents)
• Critical Illness Insurance
• Prepaid Legal/Identity Theft or ICR Bridge Identity Theft/Credit Monitoring

PTO (Paid Time Off)
• 80 hours available after completion of 1 year service (to take in 2nd year)
• 120 hours available after completion of 2 years service (to take in 3rd and each subsequent yr)
• 160 hours available after completion of 10 years of service (to take in 11th year)

Holidays
• 8 paid holidays per calendar year (8 hours of pay)
(New Year's day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Eve, Christmas Day, and 1 floater which is designated by the company)

Bereavement Pay
• 8 hours paid per shift for immediate family (24 hours maximum benefit)

Jury Duty
• Paid leave when summoned for jury duty (8 hours per shift, 160 hours maximum benefit)
